I have a medical POA for my father. Can my family come into my home and take my father and put him in a home when I disagree?

Strongly suggest you immediately consult with an attorney to understand your responsibilities as POA and what your family legally can and can not do.
If you have properly executed POA then you should read what you can and cannot do. You may need an attorney or if necessary try to get free legal advice.
If you know what nursing home they plan to use it may be possible to provide that home with a copy of the POA and tell them that you do not agree with this move.
How do they plan to pay the nursing home?
Who receives his benefits now?
Are they trying to take away his possessions/home/money or ?
An attorney would be able to help you with correct answers.
If nothing else, try talking with someone at the Senior Services Division in your city.
I think you have to consider what is in the best interests of your father. Why does you family want him to move? Do they think that you are taking advantage of him, or that you are not taking proper care of him, or perhaps that his care is too much for you do? Who is going to pay for his care in the future? Do they wish to move him because they think that he will be better off in a care home, or are they trying to take advantage of his funds or possessions?
