Contact your local United Way and see if they can help.
Be sure to tell them your entire situation and if anyone in your family has chronic health issues (asthma, etc.) there are subsets of help out when people have health issues.

The other thing you can try, because you will need help getting the items to your new home, is see if there is a church or religious organization in your town that offers assistance rehoming people (meaning help with trucks and people to move you.)
For example where I am there are religious organizations that help resettle people who came to the US as refugees, if you met their criteria, and were in Pittsburgh, they could help you.
There are also organizations of all denominations (and some that are non-denominational that work with folks who had issues with homelessness and they also do the same things....offer help getting rehomed if you met their criteria.
There are other subsets for help if you had behavioral health issues, or a life threatening illness, or a disabled child, or if you are disabled, etc. It is all about asking and seeing what is out there.
Start making calls to the United Way, churches, synagogues, homeless shelters, soup kitchens, food pantries, and such and ask for help. If someone says no, ask if they know someone.
