My my 14 year old Shih-tzu licks profusely (paws, hind quarters), and scratches his ears. I have had him diagnosed by a vet who advises dry dog food only (skin sensitive that they sell). I have always fed him high quality, limited ingredient dry dog food his whole life and have always added table foods ie: lean beef, skinless chicken breast, plain cooked veggies, sweet potatoes, or rice. I would like to exclude the dry food altogether, but need a good recipe that is not too time consuming. Please advise.

Add whey to whatever dog food he is eating. It is very good for their skin. You can also add coconut oil.
you might cut out the carbs as these are not part of a dog's traditinal diet (carnivore & all)
some people also advocate feeding them raw meat but this can be costly
aside from that giving a dog VItamin E or fish oil capsules can help when they have skin problems
also have you checked for fleas/hotspots? Also some say a vinegar rinse and bathing them more often helps with this
