I lost the info card that came with this plant. I was wondering if anyone can tell me the name of it so I can look up care tips? I've had it for over a year and almost killed it once, but my mom helped me bring it back.

I think it is a peperomia, it could be :
Peperomia glabella variegata
Peperomia pepperspot variegata
Peperomia obtusifolia variegata (obtusifolia: if the leaves have a pointed shape)
Variegata is not the important part of the name it is nearly always part of the name of plants which have different color zones on their leaves or stems, like the yellow or white on your plant's leaves. Most of the time you will find the same plant with the same name, less the adding of "Variegata", in this case, the other plant has plain green leaves. Natural variegation is very rare in nature, it means that the majority of plants you buy which are ''variegated'' are "cultivars" which means they have been obtained by selection and that sometimes the new plants you obtain from cuttings of these plants will not be variegated and have plain green leaves.
