A few years ago, my cousin lost this monkey in Chicago IL. He had been his travel companion for many many years. I am just trying to replace it for him as a surprise. There were no tags on it the last time I saw it before he lost it. And understandably, without tag information it may be nearly impossible. But maybe someone might have information, and any information would be very helpful, as I have been searching so many sites, and google for years without any luck.

Try social media outlets! so many people online now. Place your picture on, along with your story. Someone is bound to have the answer!
Check either under Nat and Jules or Children's Palace. The face with embroidery mouthis unusual and reminds me of those brands. Unfortunately most sellers don't list brands when the sell so it is going to be hit or miss just matching photos to open auctions or sales.
You can do periodic reverse image search and see if one comes up as a match on eBay or Etsy.

I don't think they are making these anymore. I know with a good photo like the one you have right here you can get one made from him. This site is fantastic for making replacement animals for children and they even take a kids drawing and can make up the animal. Give them an email with the image and see what they tell you. www.budsies.com/
