I am making a McCalls dress and the size I ordered is too small, so I want to cut it out a bit bigger. It looks like each size up is only 1/4 inch. I only have up to 14, but I need a 16. Since McCalls seems to run small, should I add an inch or inch and a half? I have never done this before, so thought I would ask if anyone has?

In my personal opinion, this is one of the most difficult alterations to do - and get it right..
Of course, if you just have a simple sleeveless pattern then go for it, but if every piece has to be enlarged then I would advise you to bite the bullet and order another pattern - in the correct size so you will maybe have a nice fitting dress when you finish. Would it be possible to exchange the pattern you have already purchased?
If you have altered patterns/clothes before you may be able to follow the instructions given on YouTube but you should review several before making that first cut.
To do this correctly you will need several 'tools' but these can be found at most craft stores and, usually, instructions on enlarging patterns.
The following video is one of the best that I have seen. Try watching this one and see if you still feel you can enlarge your pattern so you do not have to discard something that does not fit correctly.

This instructor has several very good instructional videos on sewing and several on enlarging each part of a pattern.
You need to know what areas of your body are larger than the pattern. It may not be every area. Take your measurements and compare to the pattern.
Here is a good YouTube video on the subject www.google.com/
