My question is, if I am cleaning four homes a week, would I have to get a business license?

Ordinances vary from city to city. You can find city government for your city online. There should be a sub heading for business license. Some cities have a Small Business Liaison to helps small business owners get started or assists with concerns or problems.
Can't answer your question as it depends upon your location-county, city, area etc. You should contact your local area agencies for license requirements.
If you advertise your business, or do word of mouth are 2 different types. If you have the same clients every week, I would say no. If you help an aging adult do their home monthly, I would say no. If you want to branch out in businesses, the department of health permit would be needed as well. What kind of cleaners used, equipment.
