Lilacs produce new leaves and the clusters of flowers on the newest wood...the outer ends of the branches. Unless you are planning on collecting seeds to try to grow new lilac bushes, you can cut off the spent flower clusters after they are done blooming. It won't really hurt the bush to leave them, but cutting them off does two things: it looks nicer without the dead seed pods on there, and it helps the bush to put more energy into growing new branches and flowers next year, instead of trying to maintain growth to the seeds.

I always prune my bushes back after the growing season. It encourages new growth.

This is a difficult question as we know nothing about when you pruned it last, was the pruning done properly, how old is the tree, etc.
Here is a good link to illustrate proper pruning:

Your best bet for a correct answer would be to contact your local county extension agency as they answer questions like this but it would be best to include a couple of pictures of the tree as well as the branches. They will answer for your area but they may also want answers to other questions.

To understand what's going on with your lilac tree, we need more information. Do you prune your tree annually and how long are the branches of your lilac tree? Is your tree old? A photo of your tree would be very helpful. Are you applying enough fertilizer? After heavy pruning, it is important to fertilize the lilac to help it recover.

Your lilac tree may only have leaves near the end of branches because your tree has long branches and is lacking in nutrients. When pruning, you need nip off the long stems. Tipping encourages lilacs to sprout new healthy, bushy growth near the cut.

Proper nutrition at a constant rate will keep your seedlings growing. Plant protection and care products help keep plants healthy by preventing problems before they occur.
