I have a Kenmore sewing machine model 385. 12612090. The needle will not go up and down and the tension on the wheel is very tight. Can you suggest how to resolve?

I would consult the owners manual and give it an oiling.
First thing I would do is to see if any thread, string, yarn, fabric pieces are caught up around the wheel or the drive belt and motor, also check the bobbin housing for tangled thread. Give all moving parts a good cleaning and oil what should be oiled. That may be all that's needed to get your machine working properly. I have a pin cushion on the right end of my cabinet machine near the motor & belt, and occasionally the thread from a needle stuck in the cushion gets caught up on the motor shaft around the belt.
Normally the needle is moved up or down by the wheel on the right side of the machine. If the wheel is hard to turn this and does not roll freely then there could be an isue with the belt or the machine needing oil. Yes you are right the smaller wheel needs to be tight so the larger one can trun freely. Check to see if the wheel is turning freely and if not then take a look at the belt and make sure the machine is oiled properly.
