I am looking for a lentil soup recipe that was in the Weight Watchers materials in the late 1980s and/or 1990s. It was in one of the booklets and had zucchini in the recipe.

I would substitute lentils for the cauliflower in this Weignt Watchers recipe. simple-nourished-living.com/
I'm not sure but it sounds like you may be referring to the WW Lentil soup recipes that were really popular and called "Zero Point Soups".
There seems to be several version so you may have to look at some of the recipes to see if one matches what you are looking for..
Generally these soups were a combination of items on the WW's list of 0 points and could be added or subtracted according to what you had on hand.
Some people used the cabbage soup recipes and substituted zucchini in place of cabbage.
The following have different versions but you can see they are all basically the same but just adding or subtracting different vegetables.
Most can be cooked in a slow cooker.
Sometimes you have to go down the site to find the recipe.
