My 12 year old Cairn Terrier girl has been peeing on accident in our house once a day at around 9 pm at night, for about 2 nights. What should I do? Should I be worried?

Older dogs can have issues with incontinence. The vet can see if there are physical issues. You may have to diaper her at night.
The easiest thing to do for one or two nights before talking to the vet, is just to try to take her out around this new pee accident time and see if it resolves the issue. This fixed my sister dog's issue as she is just getting older and can't hold it as long. Now instead of X2 walks, she gets a 3rd pee break at 10 pm. That was an easy fix.
That said, if she is still peeing inside after going out, this is vet worthy as inconvenience can be a sign of other issues. Many are super easy to fix with meds or even diet change.
If you have to go the diaper route, stay away from dog diapers and use human ones (cutting a diamond hole for the tail). They don't leak, wick the pee away from the dog and are generally MUCH less expensive than pet diapers. Our Carmel used them for 2 years and was fine with them.
She was 11 pounds and wore a human size 1 (Walmart's were the best buy). A 20 pound dog would probably wear a size 3.
If you have anyone in the family with a baby, it is best to test one before buying a big bag to get the right fit and make sure the dog is tolerant. We trained Carmel with treats to get her used to diapie time.

To me it seems most likely you are dealing with an aging issue. It is fairly common at her age. But you so need to see your Vet to rule out other possible issues. At age 12 she should be seeing her Vet for a bi- yearly or yearly checkup.
