Your local American Red Cross office provides information and training for lifeguard and other goals. You can access this website for further information:

Each state has different laws. I have taught the medical side of the class in the past, here and in the next state over, and the requirements are different for each. Check with your local park and rec department or American Red Cross.
Check with your potential employer as my daughter's employer provided the required training and provided transportation as well as covered the fee.
You can also go to the place you are interested in lifeguarding. They should have information too
Every state is different, but the basic rules and training are probably close to the same.
A private company (such as a hotel or motel) will most likely require a state certified lifeguard.
