I got it at Whole Foods 9 years ago. I overwatered it after I was on vacation for a few weeks and killed it. :( Other than that incident it was the easiest to care for. I'd like to get another one if I can find one.

I believe it is a Calathea. There are many varieties. I think yours may be a Zebra Calathea. (sometimes called a Prayer Plant). Perhaps with this clue you can search the web and find out.
The leaves look like a Begonia of some sort. Did it grow very tall? If so I'd say it's a Begonia Rex.

I would also say a Prayer Plant, especially if the leaves close up somewhat at night
