The following is a copy of part of my post to Nora, who created the thread, "Internet Point Programs," which is also on this site. I believe it will answer your question, too.
My favorite is It Pays to Learn. The first thing you have to do each month is earn 500 points from taking quizzes. There are a couple dozen subjects and this only takes about half an hour or so. I've only been a member since March 30, 2004 and have earned more than $100. That is cash money which has been deposited into my PayPal account, that I've transferred to my personal checking account and have spent on bills and to buy gifts. The link for IPTL is:
My second favorite site is one that is relatively new to me. It's called Six Questions. One can earn points by logging in, taking opinion polls, wagering points on quizzes in the hopes you'll get the questions correct and earn more points, wagering points to play games, submitting quizzes for which you'll earn points if they are used, etc., etc., etc. Some of the current quizzes are Upside Down, Reality TV, Which doesnt belong?, Hershey Candy, Strange Phobias, The dog groups, Spooky Tunes, Its Beer Thirty and Woodstock! There are half a dozen or so programs (Lucky Points, Memo Link, PCH Points, Savings Register Rewards, My Points, Free Ride) where I have to send you an invitation directly to your e-mail address. I don't trust any site that requires my credit card to send me my prizes and some things aren't worth the shipping and handling added. That's why I decided not to sign up with six questions even though it looked like fun.

I'm confused as to where you get the information that Six Questions requires your credit card information in order to send you a prize. Also, I don't see where you found anything about paying S&H. The following is all the info I can find about redeeming points for prizes and it doesn't say anything about S&H.
SixQuestions Prize Redemption information
All prize redemptions will be shipped or emailed within 2-6 weeks.
All prizes will be shipped from the company where the prize is originally purchased from.
When you redeem points you will receive a prize redemption email from us and when your prize redemption has been fulfilled you will receive another email.
Before redeeming points for prizes, SixQuestions verifies that all personal information given throughout the duration of a membership is valid and true. If it is found that any personal information given is false then the account will be terminated.
If after six weeks of ordering a prize you have not received anything you can email us at prizes@sixquestions.com and we will reply with your prize status. Please wait six weeks before emailing us regarding your prize order.
The prizes range from $10 gift cards at places like Wal-Mart and Amazon, to a Hinckley Picnic Boat - all prizes I would gladly pay S&H for. Perhaps you encountered this while completing one of the offers in the Earn Points section?
