Where can I find a blue short sleeve, white collar men's dress shirt for a lollipop kid costume?

Try ebay or you could cut off a white collar off of one shirt and sew it to the blue shirt.
If you have a Marshalls store in your area, try there. I work at Marshalls here in GA and we carry them.
