Google "simple machines", and look for a site caslled MIKIDS.com. It gives a simple explanation and pictures of simple machines.
A catapult? Better check on the simple machine requirements first!
I have a little 'machine' that I made. It is a paper clip bent into a crank handle, with the narrow end spread open a little to fit into the hole of a music cassette. It is used to rewind the cassette when the player isn't available.
You could help your 5th grader to build a yarn winder, using the same sort of crank-handle theory.
